Thank you for your interest in registering! We will be notified as soon as you have registered via the website. We will then register you within a few days or you will be put on the waiting list. When the registration has been processed, you will receive a confirmation email (also keep an eye on your spam box!). From that moment on we are your doctor. Then do not forget to unsubscribe from your old GP so that we can receive your medical file.

Welcome to the Zomerlinde general practice, we have a pause in the registration process, and will be able to welcome new patients in the course of June.
Do you want to register multiple family members?
Then complete the form separately for each family member. Provide a different email address for each family member.
Children also need to have a UNIQUE EMAIL ADDRESS to create their own portal. This may be an alias of another email address. The telephone number may be shared with multiple family members.
Are your family members 16 years or older?
They must give permission themselves to be registered.
Practical Information
All consultations by patients with Dutch insurance are invoiced directly to the insurer. You can find the rates on the website of the Dutch Healthcare Authority, the Nza.
